To ensure your tax return is prepared accurately and in compliance with IRS regulations, please review and gather all applicable documents listed below before completing your intake form.

Identity Verification Required
- ✔ Valid, unexpired government-issued photo ID (Driver's License, State ID, Passport)
- ✔ Social Security card or official SSA document
- ✔ Spouse ID & SSN (if filing jointly)
Dependents
- ✔ SSN card for each dependent
- ✔ Birth certificate (recommended for new dependents)
- ✔ Proof of residency (school, medical, or daycare records)
- ✔ Proof of relationship (if not biological)
- ✔ Adoption papers
- ✔ Court-appointed guardianship documents
- ✔ Other official legal documents showing parental relationship
- ✔ Proof of residency (if child is not biological)
- ✔ School records showing child’s address
- ✔ Medical or daycare records listing child at your address
- ✔ Government correspondence addressed to the child at your residence
Income Documents
- ✔ W-2s from all employers
- ✔ 1099-NEC / 1099-MISC / 1099-K
- ✔ SSA-1099 or 1099-G (Social Security / Unemployment)
- ✔ 1099-INT / 1099-DIV / 1099-R (Investments / Retirement)
Self-Employment
- ✔ Income summary
- ✔ Business expenses documentation
- ✔ Vehicle mileage or business expense records
Credits & Filing Status
- ✔ Childcare provider name, address, EIN/SSN
- ✔ Household expense proof (Head of Household)
- ✔ Health insurance form 1095-A (if applicable)
Prior-Year & IRS Info
- ✔ Prior-year tax return
- ✔ IRS letters, notices, or CP messages
- ✔ IRS Identity Protection PIN (if issued)
